Forms

Forms that do the arithmetic.Built once, filled in anywhere.

Build the form your work needs from fourteen question types: sections and repeatable fields, answers from short text to photos and signatures, and calculations and risk matrices that score as you go. Questions show only when they apply, findings raise records on the spot, and every published version stays exactly as it was.

  • 01

    Structure, answers and scoring

Sections and repeatable fields shape the form. Short and long text, numbers, decimals, yes or no, dates, single and multiple choice, photos and signatures take the answers. Calculations and risk matrices do the scoring.

  • 02

    Settings where you need them

Required, help text, minimums and maximums, comments and photos on any answer, and which records can be added from it, all beside the question you are editing.

  • 03

The tree tells you what is going on

Each question shows its type, whether it is required, a summary such as "4 options, scored", and markers for comments, photos, records and logic.

Repeatable fields · Entries

Questions that repeat for every item

A repeatable field is a set of questions answered once per entry, with as many entries as the visit needs: every asset, room, location or item.

  • 01

    As many entries as needed

Set a minimum and maximum, and name what each entry is called, so the form reads in your words.

  • 02

    Anything inside

Choices, numbers, photos, calculations, even another repeatable field, answered fresh for each entry.

  • 03

    A table in the report

Entries with a few short answers print as one table, a row per entry, instead of page after page.

Scoring · Worked out live

Scores, totals and bands as you go

Options carry scores, so calculations can add, average or weigh them, or total them across every entry, and put the result in a band with your own name and colour.

  • 01

    Combine questions

Add up, multiply, average, or work out a percentage of the most possible, with a weight on each question.

  • 02

    Across entries

Add up, average, the lowest or highest entry, or count the entries, optionally only those in one band.

  • 03

    Overrides that explain themselves

A band override sets the band when an answer calls for it, such as any asset marked unsafe, and the result says why with a "Because" line.

Logic · Risk · Records

Questions that show when they apply

Logic shows a question only when earlier answers call for it, a risk matrix rates two factors on a coloured grid, and findings can raise records on the spot.

  • 01

    Logic on answers and bands

Show a question when an answer is yes or no, is or is not one of some options, equals or passes a number, is answered, or when a calculation lands in a band. Match all of the rules or any.

  • 02

    Risk matrices

Two axes of your own, such as likelihood and severity, scored and banded, with the result shown on the grid.

  • 03

    Records from an answer

Let an answer add records, such as actions or defects, already linked to the record being inspected, and kept as they were once the work is complete.

Versions · Draft, Live, Previous

Change the form, never the finished work

Edits always go into a draft. Publishing makes it live for new assignments, and every assignment stays on the version it started on.

  • 01

    Draft, Live and Previous

Each form keeps its versions together, with who published each one, when, and a summary of what changed.

  • 02

    History stays historical

An inspection filled in on version 2 reads, and reports, exactly as version 2 asked it, after you have moved on to version 4.

  • 03

    Required means required

Moving an assignment into a locked status checks every required question that applies has an answer first.

FAQs

Questions about forms

What happens to finished work when I change a form?

Nothing. Edits go into a draft, and publishing it only affects assignments started afterwards. Every assignment stays on the version it started on, and its answers and report read exactly as that version asked them.

Can a form work out a score?

Yes. Options carry scores, and calculations can add, multiply or average questions, work out a percentage of the most possible, or total a repeatable field's entries, then place the result in bands you name and colour.

Can questions appear only when they are needed?

Yes. Logic shows a question only when earlier answers match the rules you set, including when a calculation lands in a band. Hidden questions are never required.

Does the same form work on the phone?

Yes. The mobile app fills in the same forms, with repeatable fields, calculations, logic, risk matrices, photos and signatures, with or without signal.

Who can build and publish forms?

Anyone whose role has the Manage forms permission. Everyone else fills them in.
