Building and publishing a form in Trysquare

Create a form, choose the models it is used on, add sections and questions, show questions only when they apply, and publish versions safely.

A form is the set of questions your team answers on a record: an inspection, a check, a survey, a visit report. You build it once, choose which models it is used on, and publish it. Each time someone carries it out on a record, that is an assignment.

Permission needed

You need the Manage forms permission to build forms.

Creating a form

  1. Open Admin settings from your workspace name at the top of the sidebar, then choose Forms under Configuration.
  2. Click Create form.
  3. Give it a Name, and a Description if it helps, then click Create form.

Trysquare opens the new form in the builder.

Choosing where the form is used

A form only appears on the records of the models you choose. Until you choose some, the builder reminds you that it is not used on any models yet.

  1. In the builder, open the More form actions menu (the three dots) and choose Settings.
  2. Under Applies to, tick each model the form can be used on.
  3. Save the settings.

You can change these at any time without publishing a new version.

Adding sections and questions

On a wide screen the builder has three panels: the question types on the left, the form's Questions in the middle, and the selected question's settings on the right. On a smaller screen, use the Add menu instead, and settings open in a side panel.

Drag a question type into the form, or select one to add it after the selected question. The types are:

GroupTypes
StructureSection, Repeatable field
AnswersShort text, Long text, Number, Decimal, Yes / no, Date, Single choice, Multiple choice, Photo, Signature
ScoringCalculation, Risk matrix

A Section is a page of the form, and can hold questions, other sections and repeatable fields. Questions can be dragged to reorder them or move them between sections.

Setting up each question

Select a question to edit it. Changes to a question are held until you click Add (for a new question) or Save.

Basics holds the Title, optional Help text shown to the person filling it in, and Required.

The type's own settings follow:

  • Short text and Long text: a maximum length.
  • Number and Decimal: a minimum and maximum.
  • Yes / no: a colour for yes and for no.
  • Single choice and Multiple choice: the options, each with a label and optionally a score, a short code, a colour and a description. Multiple choice can also set the fewest and most selections.
  • Photo: the most photos, and which image types are accepted.

Most answer types can also allow extras on the answer, under Annotations:

  • Allow comments, for a note alongside the answer.
  • Allow images, for photos as evidence.
  • Allow records, so the person answering can create records from the answer itself. For example, an answer that finds a fault can raise an Action record there and then. You choose which models can be added, how many, and whether to prefill them with the record being inspected.

Showing questions only when they apply

Under Logic, a question, section or repeatable field can be shown only when earlier answers match. Click Add condition, pick an earlier question and what its answer should be (such as is yes, is any of, is more than or is answered), and choose whether all or any of the conditions must match. Conditions can only use questions that come earlier in the form.

Publishing

A new form is a draft. When it is ready:

  1. Click Publish.
  2. Add a Change summary if you want a note of what changed.
  3. Confirm.

Publishing makes this version live and locks it. A live form cannot be edited directly. To change it, click Create new draft version: the draft starts as a copy of the live version, and the live one stays in use until you publish the draft.

Versions protect past work

Each assignment is pinned to the version that was live when it started. Publishing a new version does not change assignments already under way, so an inspection carried out last spring still reads exactly as it did then.

The forms list shows each form's status and version. Expand a form to see its version history, with who published each version and when.

Things to know

Forms cannot be duplicated or archived yet.

Deleting a form removes it from the list, and cannot be undone.
